Services and Self-Pay Policy

Payment is due at the time of service

Superbills may be provided upon request, but reimbursement is not guaranteed
All fees are the responsibility of the client.
Scheduling, Cancellations, and No-Shows
Appointments must be cancelled or rescheduled with at least 24 hours’ notice.
Late cancellations or missed appointments may be subject to a cancellation or no-show fee. These fees are not billable to insurance and are the responsibility of the client.
Assumption of Risk
Physical therapy, exercise, and movement-based services involve inherent risks. By participating in services provided by Pelvic Pilates Physical Therapy LLC, you acknowledge and accept these risks.
